The easiest way to incorporate appropriately formatted problem lists into clinical documentation is to take advantage of AHS standardized templates made available via the "Notes" link in the Sidebar index of opened charts:
The templates ensure that problem lists appear in the right place and in the right way (e.g., discharge summary problem lists need to include problems resolved during the hospital encounter). Adoption of problem-oriented charting workflows takes care of problem list formatting automatically.
Clinicians may want to insert problem lists into blank notes or personalized SmartPhrases. A listing of all AHS-validated problem SmartLinks is provided in the Connect Care Manual (search for "Smart Stuff"). Look for the link to "Problem Management SmartTools", where there is a listing of SmartLinks together with descriptions and indications of the care contexts in which they should be used.
